How do places that give people dental insurance determine whether or not someone has a preexisting condition?

how do they know if the person taking out the dental insurance has a pre - existing condition or not ?

Well, your new dentist can tell them right off. He’ll note any noot canals, cavities, work you’ve had done. If he finds a cavity right off, that’s a pre exisitng condition.
The insurance companies only care when you file a claim for a pre-exisitng.
There are several ways. For instance if you need a bridge to replace tooth, when the dentist sends in a pre op x-ray, they can see if you already have a tooth missing. Many dental insurances don’t have a pre existing clause.
